of standard proof of age of Life Assured
- Extract
from the municipal or other records made at the time of birth
- School
or college certificate or authenticated extract from the school
or college records, if the date of birth is stated therein. A bonafide
certificate, whether recent or old, from a school or college can
be accepted as proof of age provided the following are adhered to:
- The
certificate/extracts should specify the date of birth.
- State
that the date of birth is extracted from the school/college records
Stamped and signed by the college/school
- Extract
from service register in case of:
Government and semi-government employees
- Commercial
institutions and industrial undertaking provided it is specifically
mentioned in such extract that conclusive evidence of age was produced
at the time of recruitment of the employees
- The
extract should be on company’s letterhead and the company’s
authorized signatory should sign it and company’s seal should
be affixed.
- In case
of defense/central government/ state government personnel, identity
card issued respectively by the defense department /central government/
state government to their personnel showing, inter alias, the date
of birth or age.
- Marriage
certificate in the case of Roman Catholics issued by Roman Catholic
Church
Domicile certificate in which the date of birth stated was proved
on the strength of the school certificate or birth certificates.
- Passport.
- PAN
(Permanent Account Number) card issued by the income tax authorities
Driver’s license issued by the RTO can be accepted as Age
proof provided that the driver’s license should have been
help by the life to be insured for more than 1 year. In other words
the “date of issue” on the drivers license be at least
2 years prior to the date of application for insurance.
- Ration
Card, which specifies the Date of Issue of the Ration Card and the
Date of Birth or Age of the Life to be Insured.
- Election
ID card (also called voters ID) issued by the Election Commission
of India can be accepted as valid age proof provided it was issued
at least 2 years before the date of the insurance proposal
- Copy
of LIC policy would also be considered as standard age proof, only
if the policy documents bears the date of birth and age admission
is as per the codes listed below:
- P: Passport
- S: School

- C: College
certificate
- F: Service

- M: Municipal
birth certificate
- B: Baptism

ID card
- J: Domicile

certificate in case of Roman Catholics
Where
LIC Policy Document does not have a code, copy of LIC policy for following
plans of LIC (where only standard age proofs are taken) will be accepted
as valid age proofs:
